Job summary

Inspired Homecare in Chicago seeks a Home Health Clinical Supervisor to oversee patient care and clinical staff, ensuring care is coordinated, timely, and aligned with individualized plans of care and agency standards. The role includes supervising RN/LPN, OT, PT, and HHA; coordinating care and referrals; and ensuring ongoing assessment of patient needs while maintaining regulatory compliance.

Qualifications

• RN with an active state license
• 2–3 years of home health experience required
• Strong leadership, communication, and problem-solving skills
• CPR certification and a valid driver's license

Responsibilities

• Supervise all patient care services and staff (RN, LPN, OT, PT, HHA)
• Coordinate patient care and referrals
• Ensure ongoing assessment of patient needs
• Provide direct patient care when staffing requires
• Support clinicians with OASIS, documentation and case management
• Review clinical documentation
• Lead case conference, clinical meetings, and in-services
• Ensure patient charts and records are accurate, and regulatory compliant
• Monitor clinical outcomes

Qualifications

• Registered Nurse (RN) with current state license
• 2-3 years of home health experience required
• Strong leadership, communication, and problem-solving skills
• CPR Certification, valid driver's license and auto insurance

Compensation

• 93-98 per year $93-98k annually
• Full Time
• Health, Vision, Dental, Short term and long-term disability, 401K option

Sign on Bonus: $1250, after the first 90 days, and another $1250 after a year of employment
