Job description

This is a hybrid role split between Steward House residential and the outpatient program, located at Silver Hill Hospital in New Canaan, Connecticut. The Clinical Social Worker carries a small residential caseload, supports outpatient case management, discharge planning, and group and family work, and provides backup coverage across the outpatient team so that patients receive consistent, coordinated care throughout their treatment.Steward House at Silver Hill is an 8-bed residential program located in a private residence on Silver Hill Hospital’s campus in New Canaan, Connecticut. The program is designed exclusively for executives and other professionals managing significant career responsibilities who are experiencing mental health and/or substance use problems, including anxiety, depression, mood and personality disorders, trauma, grief, job-related stress, and life transitions. The program offers a confidential, immersive, six-week program with a 30-day minimum commitment, built around an individualized, psychodynamic treatment approach with a structured daily schedule of individual and group therapy. Patients are supported by a multidisciplinary care team of psychiatrists, psychologists, social workers, and counselors, using evidence-based modalities such as DBT, CBT, psychodrama, process group, and psychoeducation, alongside experiential and wellness offerings including art and music therapy, yoga, and meditation. Steward Outpatient is an extension of the residential program, providing outpatient group therapy, individual therapy, medication management, and family therapy.Duties/Responsibilities:Serves in a hybrid capacity between the Steward House residential program and the outpatient program.Carries 2–3 residential cases.Attends all scheduled Steward House meetings, including treatment team rounds, administrative rounds, orientation group, case conference, and the outpatient team meeting.Facilitates outpatient phone intakes as backup coverage.Completes outpatient psychosocial assessments as needed.Provides case management and discharge planning for assigned outpatients.Holds a 30-minute individual appointment each week with an outpatient patient requiring case management or discharge planning support.Conducts a weekly family call, as needed.Attends family meetings as they arise.Facilitates 1–2 groups per week and is available for group coverage as needed.Provides backup clinical coverage for other outpatient social workers as needed.Completes required paperwork, including disability forms, insurance paperwork, and discharge paperwork.Performs other duties as needed.Required Skills/Abilities:Excellent organizational and interpersonal skills.Strong computer skills.Ability to flourish in a fast-paced environment.Education and Experience:Master’s degree in Social Work, Mental Health Counseling, or a related field.LCSW/LPC required, or LCSW/LPC test-eligible within 3 months of application.Experience working with psychiatric patients required.Silver Hill Hospital (“SHH”) is fully committed to equal employment and advancement opportunities for all present employees as well as for applicants in all phases of the employment process (recruitment, hiring, assignment, conditions of employment, compensation, benefits, training, promotion, transfer, discipline and termination).
