Scope of Work

To work collaboratively with members of an interdisciplinary team to assist patients and families with complex psychosocial needs. These interventions may include, but are not limited to: alternate site coordination, discharge planning, addressing all forms of identified abuse/neglect, adoption, substance use disorders, end-of-life decisions, providing referral resources, bereavement support, adjustment counseling, facilitating guardianships, and managing behavioral health concerns (including psychiatric transfers).

1. Conduct psychosocial assessments with patients and/or families to identify psychosocial needs or post-discharge care requirements and develop appropriate plans.
2. Communicate effectively and promptly with alternate care sites and community agencies to address patient needs.
3. Document assessments and plans accurately in the electronic medical record, reflecting current conditions, situational factors, transition needs, and psychosocial considerations.
4. Provide consultation and resources to healthcare team members.
5. Maintain current knowledge of community resources relevant to the role.
6. Participate in or assist with department or hospital committees and projects as assigned.
7. Stay informed about federal and state regulations applicable to the role.
8. In inpatient psychiatric settings:
1. Conduct daily group therapy sessions with eligible inpatients and partial program patients, and perform utilization reviews as needed.
2. Provide discharge planning and obtain utilization approvals for post-inpatient care.

Qualifications

• Required: Master's Degree in Social Work

• Experience in individual and family assessment, crisis intervention, grief and loss counseling, and discharge planning is preferred.

• Must hold a Master Social Worker (MSW) license in Michigan upon hire, or a Master Social Work Limited License (LTD) in Michigan upon hire, and have 5 years of experience as a Master Social Worker in Michigan.
