JOB SUMMARY

*PRN Weekends and Weekdays needed!*

The PRN/PT Social Worker MSW 1 provides patients and family members with education and advice to cope with emotional or physical issues, as outlined by the Texas State Board of Social Worker Examiners scope of practice. The role includes coordinating services such as crisis intervention, bereavement support, client advocacy, referrals, prevention, education, discharge planning, psychosocial assessments, and participating in performance improvement initiatives.

ESSENTIAL FUNCTIONS OF THE ROLE

1. Conducts and documents detailed psycho-social assessments to interpret social, environmental, and medical needs of the patient.
2. Employs advanced practice methods in developing, implementing, and evaluating treatment plans.
3. Provides patient and family education; serves as a professional resource.
4. Provides or supervises interventions including resource and referral assistance, crisis intervention, prevention, education, patient advocacy, bereavement support, therapy, and counseling as appropriate.
5. Collaborates with the healthcare team and involves the patient and family in planning and implementation.
6. Acts as a liaison with community resources to facilitate patient transition and placement.
7. Participates in performance improvement initiatives, including data collection, analysis, and continuing education.
8. Prepares professional reports, letters, and case summaries.

KEY SUCCESS FACTORS

• Knowledge of human behavior, psychosocial methods, and behavioral/affective disorder treatment.
• Understanding of principles to restore or enhance social and psychosocial functioning.
• Listening and interpersonal skills.
• Verbal and written communication skills.
• Proficiency with computers and software applications.
• Ability to analyze data and assist individuals in problem-solving.
• Ability to handle grief effectively.
