Under general supervision, provides therapeutic intervention and advanced social work services to patients and their families to enhance comprehensive, integrated, and uninterrupted care across the continuum, ensuring continuity of care in the community. Acts as a resource to Clinical Social Worker I incumbents. Demonstrates positive interpersonal relations with patients, families, visitors, and co-workers in accordance with UMMS and departmental policies and procedures.

General Summary

Functions autonomously as a member of the multidisciplinary healthcare team to provide case management, therapeutic crisis intervention, biopsychosocial assessment, therapy and counseling, referral, treatment planning, and other clinical social work services within their practice area. Maintains expert knowledge of community resources to address the diverse needs of patients, groups, and families. Uses understanding of social determinants of health and clinical best practices to identify interventions that enhance patients' functioning within their psychosocial environment.

II. Principal Responsibilities and Tasks

The following statements describe the general work performed in this role. It is not an exhaustive list of all duties that may be performed.

  • Responsible for oversight of all clinical assessments performed by supervisees. Completes specialized assessments in focus areas/practice settings as needed. Provides feedback to improve assessment tools within their area.
  • Serves as the lead psychosocial expert on the healthcare team to ensure care plans are culturally sensitive and address physical, emotional, social, and spiritual aspects of care, including recommendations for emotional and behavioral health treatments. Evaluates how Advance Care Planning is incorporated into patient care.
  • Provides and models clinically advanced care within the practice setting. Offers expertise in various aspects of social work to the healthcare system.
  • Analyzes department performance effectiveness and provides feedback on specialty care interventions across the healthcare system.
  • Completes clinical chart reviews of referrals/services. Shares resources with colleagues by maintaining a current guide of relevant community resources.
  • Evaluates referral sources utilized and provides feedback regarding their impact on clinical care and patient needs. Leads networking with external agencies based on social determinants of health.
  • Provides counseling to staff regarding counseling practices and techniques. Demonstrates and models counseling approaches by working with patients with multifaceted needs.
  • Provides clinical supervision, under a written contract required by the Board of Social Work Examiners, for licensed social workers seeking supervision.
  • Offers informal and formal consultations within the healthcare system.
  • Models exemplary patient education. Leads clinical teaching and instruction of social work practices related to clinical, psychosocial, emotional, and behavioral health issues.
  • Applies various approaches to advocate for team members and supervisees. Keeps statistics on service volume, caseload, admissions/discharges for equitable work distribution, if applicable.
  • Engages in professional development and networking through participation in professional organizations.
  • Works with leadership to develop and implement policies and procedures to ensure compliance with regulatory standards.
  • Completes all specific job tasks within their department as outlined by supervisor.

Education and Experience

  • Maryland state Social Work licensure and credentials as a Licensed Certified Social Worker – Clinical (LCSW-C) are required.
  • Two years of applicable experience in a Behavioral Health setting is preferred.
  • Three years of applicable social work experience is required.
