Qualifications

Responsibilities

• Performs psychosocial screenings and assessments and integrates plan of care.
• Counsels patients and families on emotional, social, financial aspects.
• Participates in discharge planning and post-hospital placement planning.
• Educates patients/families about post-acute options and resources.
• Maintains documentation and communicates with care teams.
