Job summary

Parkland Health is seeking a licensed hospital social worker to perform psychosocial screenings and assessments and implement the psychosocial plan of care for patients, ensuring adequate resources and support services. This role requires an MSW, state licensure, CPR, and SAMA certifications.

You will work with patients, families and the care team to promote timely discharge, empowerment and connection to community resources, while documenting interventions and outcomes per policy.

Qualifications

• Master's degree in social work required.
• Licensed by the Texas State Board of Social Work.
• SAMA certification (level 1-4) required before first clinical orientation shift.
• Current Heartsaver CPR certification within the specified timeframe.
• Must communicate effectively with a diverse population.

Responsibilities

• Performs psychosocial screenings and assessments to identify patient needs.
• Collaborates with patients, families and multidisciplinary team to address psychosocial needs.
• Counsels patients and families on emotional, social, and financial aspects of illness.
• Assists with discharge planning and post-hospital placement planning.
• Maintains documentation and ensures compliance with regulations.
