Job summary

Ochsner Health is seeking a qualified Social Worker to perform bio-psychosocial assessments, education, and discharge planning in a hospital setting. You will coordinate with the multidisciplinary team to support patients and families, with supervision from an LCSW as needed.

Required MSW and CSW/RSW or LMSW preferred; strong communication, data entry, and knowledge of public programs are essential for success in this role.

Qualifications

• MSW from CSWE-accredited program required.
• CSW/RSW certification or LMSW preferred; LMSW must schedule and pass exam within 6 months.

Responsibilities

• Conducts patient assessment interviews, provides referrals and coordinates discharge plans.
• Identifies and provides helpful resources to patients and families.
• Maintains patient records and documents and communicates critical information to the medical team.
• Maintains professional and technical knowledge and required certifications through continuing education and development.
• Utilizes management resources.
• Participates in on-call rotation.
• Adapts behavior to the patient population and privacy requirements.
