Job summary

Qualifications

• Master's degree in Social Work (MSW) from a CSWE-accredited program.
• Certified Social Worker (CSW) or Registered Social Worker (RSW) per LABSWE; LMSW preferred.
• Ability to perform clinical social work under supervision if required.
• Experience in medical settings preferred (two years).

Responsibilities

• Conducts patient assessment interviews, provides referrals and coordinates discharge plans.
• Identifies and provides helpful resources to patients and families.
• Maintains patient records and communicates critical information to the medical team.
• Maintains professional knowledge through continuing education and certification updates.
• Participates in on-call rotation.
• Adapts communication and services to diverse patient populations.
