Universal Health Services (UHS) seeks a Clinical Social Worker for its Behavioral Health Unit in Atlanta. This role focuses on discharge planning, coordination of care, and post-discharge support for youths with diverse psychiatric needs.

The position requires an MS degree in counseling/psychology or a related field and an active Georgia social work license (LSW/LMSW/LCSW) or eligibility for LAAC within 6 months. Two years of acute care experience is preferred.

Formación

• MS degree in counseling, psychology or related social services field.
• Minimum of two years experience in acute care or related healthcare field preferred.
• Must hold an active license in Georgia as a Licensed Social Worker (LSW) (LMSW/LCSW) or a Professional Clinical Counselor (PC). OR ability to apply for LAAC within six months.

Responsabilidades

• Facilitate coordination of continued rehabilitation and after discharge, such as home health therapies, outpatient rehabilitation or continuing care centers.
• Support medical teams in coordination of community health care needs.
• Assist patients in locating community benefits/resources.
• Schedule and facilitate family meetings as needed.
• Track insurance benefits and authorization for inpatient rehab, updating providers and seeking additional authorization when needed.
• Participate in interdisciplinary team meetings and collaborate to support discharge planning, advocating for patient needs and preferences.
• Keep patients and families updated on discharge planning tasks and progress.
