Qualifications

• Minimum of two years experience in acute care or related healthcare field preferred.
• Active license in Georgia as a Licensed Social Worker (LSW) (LMSW/LCSW) or a Professional Clinical Counselor (PC). OR ability to apply for LAAC within six months.
• Must demonstrate knowledge of community resources to meet post-discharge clinical and social needs.

Responsibilities

• Discharge planning and coordination of care for patients in the Behavioral Health Unit.
• Coordinate services during and after admission to acute rehab.
• Schedule and facilitate family meetings and update on discharge tasks.
• Collaborate with interdisciplinary teams to support discharge planning and advocacy.
