Responsibilities

• Provides biopsychosocial assessments and clinical diagnoses with treatment planning.
• Interviews patient and support system for initial psychosocial evaluation.
• Delivers brief psychotherapy and education; assists coping with hospitalization and illness.
• Advocates for patient and family across the care continuum.
• Coordinates care with multidisciplinary team; monitors and revises care plans.
• Supervises provisional social workers and mentors new staff.
• engages in crisis intervention and population-based program development.
• Documents and gathers data; references external resources when appropriate.
