Acadia Healthcare in Columbus, OH seeks a qualified Social Worker to plan, coordinate and deliver therapeutic support for patients with mental health, behavioral health and substance use disorders.

The role includes conducting social histories, developing treatment plans, coordinating programs, promoting recovery and providing crisis management and family sessions while prioritizing patient safety and compliance with licensure requirements.

Qualifications

• Bachelor's degree in Social Work from an accredited college or university.
• One or more years’ experience working with patient population preferred.
• CPR and de-escalation/restraint certification required (training available upon hire).
• First aid may be required based on state or facility.

Responsabilités

• Conduct social history assessment of the patient through observation and patient/family interview.
• Coordinate and plan programs and activities to meet patient’s social and emotional needs.
• Promote recovery, symptom reduction and the highest level of social functioning.
• Provide crisis management and family sessions to assist the family in understanding implications and impact.
• Assist in developing effective treatment plans that ensure safety.
• Participate in multidisciplinary treatment team meetings.
• Communicate or aid family members in understanding and supporting the patient.
• Manage a caseload and patient chart documentation.
• Facilitate education, process groups, support groups and referrals.
• Demonstrate a positive, empathetic and professional attitude toward customers.

Formation

Bachelor's degree in Social Work

1+ year experience with patient population
