Job summary

Resurrection Medical Center is seeking a licensed social worker to provide psychosocial support across pediatrics to older adults. The role involves crisis intervention, counseling, case management, and discharge planning within a multidisciplinary team.

Requirements include a Master’s degree in social work or related field. At least one year of hospital or outpatient mental health experience is expected; licensure must be current where required.

Qualifications

• MSW or related degree required.
• If licensure is not required in the state, registration with State Board within 3 months of hire is required.
• If licensure is required by the state, licensure must be obtained by time of hire and maintained.

Responsibilities

• Provide emotional support, crisis intervention and counseling for Pediatrics, Adolescents, Adults, Older Adults and families.
• Provide information and referral, and coordinate referrals for financial assistance and supportive services.
• Perform case management and milieu management; complete psychosocial assessments; assist in treatment planning and discharge planning; document and maintain clinical records.
