Job summary

Intermountain Health Primary Childrens Hospital in Salt Lake City, Utah, seeks a Social Work Care Manager I to perform psychosocial assessments, develop care plans, and provide counseling and referrals in collaboration with the care team.

Qualifications

• MSW from an accredited institution; degree verification required.
• Current state licensure (CSW/LCSW) before/upon supervision hours.
• Strong written and verbal communication skills.

Responsibilities

• Assesses patients for transition planning, mental health, substance use, and goals of care.
• Develops and monitors care plans addressing social determinants and community resources.
• Provides brief therapy (individual, group, family) and psychosocial assessments.
• Identifies mental, emotional, and behavioral disorders; connects to services.
• Trains staff, educates patients, and facilitates safe transitions of care.
