Qualifications

• MSW from an accredited institution; degree verification required.
• Current state licensure (CSW/LCSW) as applicable.
• Proficiency with word processing and spreadsheet software.
• Strong written and verbal communication and critical thinking skills.

Responsibilities

• Assesses patients for transition planning and mental health needs.
• Develops and monitors care plans addressing social determinants of health and community resources.
• Provides brief individual, group, and family therapy.
• Identifies mental, emotional, and behavioral disorders and connects patients to services.
• Trains staff, educates patients, and advocates for access to care.
• Collaborates with healthcare teams, insurers, and community providers for quality care.
• Leads improvement initiatives and ensures policy adherence.
• Facilitates clinical goals of care discussions with patients, families, and teams.
