Job summary

Intermountain Health in Utah seeks a Social Work Care Manager I to perform psychosocial assessments, develop care plans, and provide brief therapeutic interventions in collaboration with care teams. This PRN, days shift role includes a $500 sign-on bonus for external applicants with 1+ year of experience.

Responsibilities include care coordination, crisis assessment, education, advocacy, and ensuring transitions of care with community resources, insurers, and providers.

Qualifications

• Master of Social Work (MSW) from an accredited institution.
• Current state licensure (CSW/LCSW) prior to completion of supervision hours.
• Basic computer proficiency (word processing, spreadsheets).
• Strong written and verbal communication skills.
• Demonstrated ability to apply critical thinking.
• Caregivers with home/community duties must maintain BLS, driver’s license, auto insurance, driving record and reliable transportation.

Responsibilities

• Assessment & Screening for transition planning, mental health, substance use, and goals of care.
• Care Coordination—develops and monitors care plans addressing social determinants of health.
• Behavioral health support using motivational interviewing and therapeutic techniques.
• Therapeutic Intervention—brief individual, group, and family therapy; psychosocial assessments.
• Diagnosis & referrals—identify disorders and connect to services.
• Education & Advocacy—train staff, educate patients, advocate for rights and access to care.
