Job summary

UAB Medicine in Birmingham, AL is seeking a Social Worker to deliver psychosocial support to individuals, families and groups facing chronic, acute or terminal illness. You will assist with education, counseling, referrals and care coordination to promote health and reduce barriers to care.

The role involves discharge planning, community resource identification, and acting as a clinical resource for students and the community.

Qualifications

• Master's degree in Social Work required.
• Work experience may NOT substitute for education requirement.
• Currently licensed as a Licensed Master Social Worker (LMSW) by the Alabama State Board of Social Work Examiners required.
• For units/departments other than UAB Hospital, certified in Basic Life Support (BLS) from the American Heart Association (AHA) is required or should be obtained within 30 days of employment.
• If job is in UAB Hospital, successful completion of First Five Rapid Response Basics online training required within 30 days of hire into position and must be maintained annually.

Responsibilities

• To provide individuals, families and groups with the psychosocial support needed to cope with chronic, acute or terminal illnesses.
• To provide services including advising family care givers, providing patient education and counseling, and making referrals for other services.
• May also provide care and case management or interventions designed to promote health, prevent disease and address barriers to access to healthcare.
• To facilitate the discharge planning process and identify community resources to assist patients and their families.
• To act as a clinical resource to both social service students and the community.
