Job summary

UCSF Health Birth Center seeks a Clinical Social Worker to intervene with patients with complex psychosocial needs, assist with social program eligibility, and coordinate discharge planning. The CSW collaborates with multidisciplinary teams to ensure timely, safe discharge and ongoing care within fiscal constraints.

The CSW III (LCSW) provides advanced clinical leadership, supervision, and community consultation, contributing to program planning and education within UCSF Health.

Qualifications

• Master's degree from a CSWE accredited school of Social Work.
• Experience (internship/career) in a healthcare and/or hospital setting preferred.
• Knowledge and abilities essential to the duties including community resources and medical/psychological aspects.
• Demonstrates resourcefulness, effective written and oral communication, diplomacy, and organizational/analytic skills.
• Self-directed, assertive and creative in problem solving and patient care management.

Responsibilities

• Coordinate discharge planning and care progression.
• Provide crisis intervention to patients and families.
• Collaborate with Case Managers, Nursing and Medical staff to optimize outcomes.
• Act as patient advocate and connect to community resources.
