Job summary

UCI Health is seeking a Clinical Social Worker to join the hospital/social work team. The role focuses on psychosocial assessment, transition management, discharge planning and resource coordination to promote optimal patient outcomes and reduce readmissions.

Qualifications

• Master's Degree in Social Work
• Licensed Clinical Social Worker in CA or license eligible
• Experience in hospital social work or medical settings
• Knowledge of Joint Commission or DHS standards and regulations
• Excellent written and verbal English communication

Responsibilities

• Assess transition needs and identify patients at risk for readmission
• Conduct complex psychosocial assessments and interventions
• Coordinate care across teams to promote timely throughput and safe discharge
• Educate patients, families and caregivers about resources and services
• Ensure compliance with regulatory requirements and Tenet policies
