Qualifications

• Master's Degree in Social Work (MSW) required.
• Knowledge of and ability to work within a complex health care system.
• Excellent written and verbal communication skills in English.

Responsibilities

• Facilitate care along a continuum through effective resource coordination.
• Assess patients for transition needs including readmission risk.
• Conduct psycho-social assessment and intervention to promote timely throughput and safe discharge.
• Ensure compliance with Joint Commission, DHS, and related standards; educate physicians, patients, families and caregivers.
