Job summary

PACS is seeking a licensed Speech Language Pathologist to evaluate patients within 24 hours of referral in a post-acute care setting. You will develop and obtain approval for treatment plans, then deliver therapy in accordance with the physician plan.

Supervisory duties include SLPA guidance and training of staff, with comprehensive documentation of progress and outcomes. You will participate in care conferences and discharge planning, ensure compliance with state practice acts, and communicate

Qualifications

• Licensed as a Speech Language Pathologist in the state.
• Masters or Doctorate in Speech Language Therapy preferred.
• Ability to read technical procedures and policy manuals.

Responsibilities

• Evaluate patients within 24 hours or 1 business day of referral.
• Develop treatment plans and obtain physician approval for services.
• Treat patients per physician treatment plan.
• Supervise SLPA in direct patient care and related activities.
• Record evaluations and daily/weekly progress notes per policy.
• Discharge planning and family/caregiver instruction for maintenance.
