Immediate Opportunity: Clinical - Senior LTSS Service Care Manager (RN)

Pay Range: $40 - $45 per hour. Benefits include health insurance (medical, dental, vision), 401(k), and paid sick leave, depending on location.

Key Responsibilities:

1. Support medical, behavioral, and social support staff in assessing needs, eligibility, and services for members.
2. Conduct screening and risk assessments according to program guidelines, including identifying members with special needs through health screens and other resources.
3. Coordinate member care with community outreach and member advocates.
4. Educate providers and community resources about program components and available support services.
5. Educate members with special needs to ensure compliance and positive outcomes.
6. Assist with care plan modifications, managing medical consents, caregivers, families, and physicians.
7. Ensure timely and compliant assessments and documentation.
8. Implement process improvements to enhance performance, member outcomes, and audit results.

Key Requirements and Experience:

• Skills: Case Management, Home Health, MCO experience
• Experience: 1-2 years in Case Management or Home Health
• Education: Associate or Bachelor’s degree in Nursing
• Licensure: Registered Nurse (RN)
• Preferred: RN with at least 1 year of experience (flexibility possible)
