Job Summary

What you will do in this role

• Act as a liaison between internal and external referral sources and rehab operations.
• Be responsible for establishing relationships with case managers, physicians, payors, and other referral sources.
• Ensure appropriate patient placement through clinical assessments and screening in collaboration with the Rehab Medical Director.
• Act as the responsible party focused on the referral and admission process of the rehab program.
• Will be required to pass facility credentialing for Functional Independence Measurement (FIM) within 90 days upon hire, then maintain current certification.

What qualifications you will need

• Basic Cardiac Life Support must be obtained within 30 days of employment start date
• Associate degree
• (RN) Registered Nurse, or (COTA) Certified Occupational Therapy Assistant, or (OTR) Occupational Therapist Registered, or (LPT) Licensed Physical Therapist, or (LPTA) Licensed Physical Therapist Assistant, or (SLP) Speech-Language Pathologist
