Job summary

Clinical Rehabilitation Specialist

We are currently looking for an ambitious Clinical Rehabilitation Specialist to join our team! A Clinical Rehab Specialist acts as a coordinator between internal/external referral sources and acute inpatient rehab operations. Your duties and responsibilities may include:

• Establishing relationships with case managers, physicians, payers and other referral sources
• Ensure appropriate patient placement through clinical screenings and assessments in collaboration with both the hospital case management team and the Rehab Medical Director
• Work closely with the hospital case management team to ensure a responsive and smooth-flowing referral and admission process for the inpatient acute rehab program

Requirements

• Graduate of an accredited school in clinical specialty
• Minimum 2 yrs. recent clinical care experience
• Licensed Professional in a clinical health care specialty (i.e., RN, Physical Therapist, Occupational Therapist, or Speech Therapist)
• Pass Functional Independence Measurement (FIM) within 90 days upon hire, then maintain current certification
• If RN, CRRN preferred and/or achieved within 2 years of hire
• BCLS certification (AHA)
• Clinical experience in the care and management of Inpatient Rehabilitation patients
• Experience as a clinical liaison coordinating admissions for Inpatient Rehabilitation patients
