Job Summary

The Clinical Registered Nurse supports the goals of the care enhancement program by providing high-quality telephonic support efficiently and cost-effectively while ensuring excellent care. The nurse coordinates with Primary Care Providers and the healthcare team to support appropriate healthcare management, resource utilization, and quality outcomes. They also identify participants at risk for increased acuity early and intervene accordingly.

Supervised by an Operations Manager, all Sharecare clinicians participate in orientation, competency reviews, and annual testing.

Available Shifts

• Monday-Friday: 7am-3:30pm CT and every 3rd Saturday 8am-4:30pm CT.
• Monday-Friday: 11am-7:30pm CT and every 3rd Saturday 8am-4:30pm CT.
• Monday-Friday: 11:30am-8pm CT and every 3rd Saturday 8am-4:30pm CT.

Training

4 weeks, 8am – 4:30pm CT. Attendance during training is mandatory for the first 60 days.

Qualifications

• Current RN license for the state of employment, with ability to obtain others as needed.
• High School diploma and Nursing diploma, with a BSN preferred.
• Minimum 3 years recent clinical experience.
• Basic knowledge of physiology and chronic diseases.
• Effective communication skills, both oral and written.
• Basic computer skills and familiarity with Windows OS.
• Ability to educate and promote behavior change telephonically.

Essential Job Functions

• Review participants' preventive health exams and initiate interventions.
• Contact participants for assessment, education, and goal setting.
• Develop and implement care plans addressing healthcare needs.
• Identify and address barriers to healthcare access.
• Coordinate with healthcare providers and team members to ensure continuity of care.
• Handle warm transfer calls for assessment when necessary.
