Job summary

Urban Pathways, Inc. seeks a Peer Specialist for the Clinical Rapid Response Team in New York, NY. The role centers on immediate crisis support, leveraging lived experience to build trust and guide clients to healing resources.

You will work with an interdisciplinary team to promote person-centered care, with emphasis on preventing crises and ensuring seamless follow-up and linkage to services.

Qualifications

• High School diploma or GED required.
• Experience using behavioral health services as a client is preferred.
• Ability and willingness to work evenings and flexible schedule.

Responsibilities

• Provide support and referrals to meet health objectives including health, mental health and substance use services.
• Offer guidance and side-by-side skills modeling to people in care.
• Respond to crises and facilitate admission when needed and ensure follow-up care.
• Develop preventative wellness plans with goals and interventions.
• Organize and lead individual and group wellness activities for high acuity clients.
• Complete incident reports within 24 hours and distribute notices.
