Job summary

WellDyne is seeking a Clinical Programs Pharmacist to lead medication therapy optimization and patient education within clinical programs. You will conduct comprehensive medication reviews, engage with patients and prescribers, and collaborate with product design teams to enhance program effectiveness.

The role requires a Doctor of Pharmacy, active license (Florida preferred), and strong skills in literature review, data analysis, and cross-functional communication.

Qualifications

• Pharmacist with PharmD and active license in good standing.
• Florida license required where applicable.
• Experience presenting to stakeholders and teams preferred.

Responsibilities

• Manage clinical programs day-to-day; schedule and support patients.
• Conduct care plans and telephonic health coaching for chronic diseases.
• Support patient and prescriber engagement for program initiatives.
• Educate providers on claims and benefit procedures.
• Develop reports on program outcomes and patient care.
• Perform literature reviews and respond to drug information needs.
• Contribute to policy and process development for clinical programs.
• Carry out medication reviews and patient consultations.
• Collaborate with nurses and health coaches to support population management.
• Communicate with patients and providers, including outbound calls.
