Qualifications

• Bachelor's degree required or equivalent combination of education and experience.
• Preferred: 1-year experience with HIV population Adult/ Pediatric outpatient clinic, NCQA PCMH and Quality Improvement Projects.

Responsibilities

• Coordinate patient services and administrative activities of clinical data and monitor ongoing clinical activities for NCQA regulations.
• Coordinate requirements and logistics for Patient Centered Model Home-PCMH annual recognition and PCMH committee meetings.
• Provide health education to patients, staff, and practitioners within area of specialty as it relates to NCQA compliance.
