Job summary

Qualifications

• Equivalent combination of education and experience may be substituted for the above education and experience requirements.

Responsibilities

• Assesses referred clients and coordinates the development of a service plan with treatment team and funding source.
• Coordinates and evaluates the service plan implementation.
• Ensures all necessary medical and dental appointments are scheduled and completed.
• Coordinates the development, review, and revision of programs and behavior intervention plans.
• Provides, monitors, and assures comprehensive support services and oversight on an ongoing basis.
• Maintains client reporting records per corporate and regulatory standards.
• May assist with mentor training, planning and development as needed.
• Travel as needed.
