Job summary

Roots Community Health Center is seeking a Clinical Programs Specialist to drive care coordination, patient education, and proactive outreach for South Bay patients. You will manage a large patient panel, coordinate visits, labs, and referrals across care teams and partner organizations.

You will document all interactions in multiple electronic tracking systems, ensure compliance with reporting requirements, and support quality improvement efforts to improve community health outcomes.

Qualifications

• Strong interest in healthcare, social determinants of health, and health disparities.
• Experience with low-income communities of color.
• Excellent written and verbal communication skills.
• Excellent team-player capabilities.
• Ability to manage multiple phone lines, spreadsheets, and files.
• Ability to work effectively with patients from diverse backgrounds.
• Strong working knowledge of Microsoft Office and G-Suite.
• Ability and willingness to learn Electronic Health Records and multiple reporting systems.
• 1 year of experience performing clinical, coordination, and/or administrative functions.

Responsibilities

• Drive care coordination across care teams and clinics.
• Coordinate visits, labs, and referrals; track patient progress.
• Provide patient education and proactive outreach.
• Document interactions in EHR and tracking systems.
• Support quality reporting and MAAs where applicable.
• Participate in team meetings and cross-functional initiatives.
