Job summary

Metropolitan Jewish Health System, Inc. is seeking a Clinical Program Liaison to evaluate and establish care plans for hospice patients referred for services. You will work on-site at a major medical center as part of the business development and marketing team.

You will build and maintain positive relationships with physicians, social workers, case managers, and discharge planners to coordinate comprehensive hospice care and ensure smooth patient transitions.

Qualifications

• Bachelor's degree in Social Work from an accredited school.
• MSW / LMSW / LCSW certification preferred.
• Minimum two years' experience in Medical-Surgical or Community Health; hospice experience preferred.
• Proficient with Microsoft Word and Excel.
• Flexible hours to meet position needs and coordinate interdisciplinary team resources.

Responsibilities

Our multidisciplinary teams offer culturally-sensitive, end-of-life and palliative care services to patients and their families at home or in assisted living communities, skilled nursing facilities and hospitals.

Working on-site at a major medical center, the Clinical Program Liaison is responsible for evaluating and establishing a plan of care for patients referred for Hospice Care services. As a key member of the business development and marketing team, the Clinical Program Liaison develops and maintains positive relationships with physicians, social workers, case managers and discharge planners within the medical center.

• Bachelor's degree from an accredited School of Social Work; LCSW, MSW or LMSW
• A minimum of two years' experience in either Medical-Surgical or Community Health. Hospice experience preferred.
• Microsoft Word and Excel
• Ability to work flexible hours to meet the needs of the position
• Ability to coordinate the resources and personnel involved in a comprehensiveinterdisciplinary team environment
