Qualifications

• BLS certification within 30 days – American Heart Association (AHA).
• Licensed Physical Therapist (PT) in good standing.

Responsibilities

• Demonstrates ability to perform patient transfers with assistance as needed from bed, chair and floor.
• Maintains safe and appropriate body mechanics to reduce injury risk.
• Designs an appropriate plan of care with goals, treatment, outcomes and discharge plan.
• Educates patients, families, caregivers and other staff using effective teaching methods.
• Evaluates clinical findings to determine PT diagnoses and outcomes of care.
• Performs examinations and selects reliable methods relevant to chief complaints and history.
• Documents in EMR and communicates progress to referring providers.
