Job summary

Mount Sinai Medical Center in South Florida is seeking a licensed Physical Therapist to join our patient-focused team. You will perform patient assessments, develop and adjust treatment plans, and document progress to support high-quality care.

The ideal candidate holds Florida PT license, CPR certification, and a Doctoral or graduate degree from an accredited program, with willingness for further specialization.

Qualifications

• Current Florida State PT License.
• CPR Certification.
• Graduate of an accredited Physical Therapy Program in accordance with state licensing requirements.
• Taken or is willing to take postgraduate education.

Responsibilities

• Initiates and completes patient assessment following departmental guidelines to determine treatment and completes documentation.
• Performs thorough assessment of patient’s functional conditions to set plan of treatment with goals.
• Updates patient’s progress and goals with appropriate documentation.
• Documents charges with each initial evaluation and daily treatment.
• Completes Outcome Measure scores for admission, goal setting and discharge in a timely manner.
• Modifies treatment based on patient response and reassesses outcomes.
• Fits, trains and educates patients in use of assistive devices and ensures equipment is ordered and measured.
• Provides written instructions and education to patients/family in a timely manner.
• Gives in-service to staff and supports clinical education programs.
