Qualifications

• Graduate of a CAPTE-accredited Physical Therapy Program.
• New graduates encouraged to apply.
• Current Tennessee PT license.
• CPR certification.
• Ability to work autonomously.
• Proficiency with EMR, database software and MS Office.

Responsibilities

• Assess patients to develop and initiate treatment plans based on evaluation results.
• Review physician referral and medical records to determine needed PT treatment.
• Provide direct patient care by established protocols and guidelines.
• Document findings, progress, and instructions to patients and caregivers.
• Oversee care delivered by assistants and rehabilitation technicians.
• Create written plan of care, set goals, and develop a comprehensive program.
• Implement PT treatment program and essential therapies.
• Communicate with patients, caregivers, physicians, and insurers.
• Ensure timely documentation including daily notes, progress reports, and discharge summaries.
