Job summary

Advocate Health in North Wilkesboro, NC is seeking a licensed Physical Therapist to provide patient care assessments and treatment in a comprehensive rehab setting. You will document evaluations, progress notes, and discharge summaries while maintaining productivity standards and contributing to department goals.

A BS in Physical Therapy is required, with MS or DPT preferred, and CPR certification renewed every two years.

Qualifications

• Bachelor of Science degree from an accredited physical therapy school required.
• M.S. or D.P.T. preferred.
• Licensed to practice Physical Therapy in the State with current licensure
• CPR certification required every two years.

Responsibilities

• Provides patient care assessment and treatment. Ensures patient evaluations and treatments are comprehensive, functional and according to the Standard of Practice.
• Maintains accurate patient documentation. Completes written patient evaluations, progress notes, and discharge notes per policy.
• Meets department financial and productivity standards. Maintains productivity standards and adjusts schedules to meet patient/department needs.
• Participates in process management to optimize quality and efficiency of services.
• Promotes professional development and education of other health care workers and provides representation on committees.
• Supervises physical therapy and physical therapy assistant students to develop clinical skills and ensure high-quality care.
• Communicates professionally with patients, physicians, staff, and external customers.
• Provides appropriate patient care in accordance with age/developmental guidelines.
