Qualifications

• Graduate of an ACPE-accredited School of Pharmacy with a BS or PharmD (required).
• Advanced pharmacy degree – Doctor of Pharmacy, Masters, Residency (preferred).
• Licensed pharmacist in the states where practicing (required).
• PGY2 or 4 years of equivalent practice experience; Board Certification in associated area (preferred).
• Exhibit proficient use of computer hardware and software (required).

Responsibilities

• Provide comprehensive pharmaceutical care to a focused population and act as a subject matter expert.
• Lead clinical programs to improve patient care and optimize medication therapy.
• Perform medication reviews and therapy management for safe and effective use.
• Identify cost-containment efforts for medication utilization and resources.
• Collaborate with pharmacy team and other health professionals to deliver high-quality services.
• Provide medication education to patients and staff; serve as a preceptor as assigned.
