Job summary

The Veterans Health Administration, Department of Veterans Affairs, is seeking a Clinical Pharmacy Specialist (Anticoagulation/Cardiology) to provide advanced pharmaceutical care in anticoagulation and cardiology clinics. The CPP will work independently within the scope of practice defined by the medical center to optimize patient outcomes.

Responsibilities include direct management of anticoagulation therapy, medication therapy management, and education for patients and staff.

• Divide time between Anticoagulation service and Cardiology with focus on guideline-directed medical therapy for heart failure.
• Coordinate, develop, manage and control the anticoagulation clinic practices; monitor anticoagulants and supervise staff.
• Independently manage anticoagulation therapy in assigned patients; provide education to patients and staff.
