Job summary

Broward Health North in Deerfield Beach is seeking a clinical pharmacy specialist to perform specialized activities within designated programs to optimize patient outcomes. Responsibilities include anticoagulation pharmacokinetics, dose adjustments, order verification, and IV to PO conversions.

The pharmacist will precept residents and students, participate in committees, and represent pharmacy across hospital and ambulatory settings in the focus area.

Qualifications

• Education: Degree from an Accredited College of Pharmacy.
• Experience: PGY1 Residency or 3 years of relevant experience.
• Licensure: Registered Pharmacist in the State of Florida.
• Certification/Registration: Board Certification or Residency preferred.
• Special Training: Demonstrates knowledge and understanding of the care needed for the specific patient population in area of assignment.
• ACLS: as needed
• PALS: as needed
• BLS Provider: from the American Heart Association or American Red Cross. We can only accept a BLS card from these two providers.

Responsibilities

• Provide specialized clinical activities related to designated clinical programs to optimize patient outcomes.
• Perform medication utilization activities including anticoagulation pharmacokinetics, dose adjustments, order verification, and IV to PO conversions.
• Precept pharmacy residents and students; participate in hospital committees and represent the department of pharmacy across hospital or ambulatory settings.
