Qualifications

• Graduation from an accredited College of Pharmacy with PharmD or equivalent.
• Maryland pharmacist license required; CPR certification active.
• Residency in area of practice preferred but not required.

Responsibilities

• Promote safe, effective medication use for patients in value-based programs.
• Coordinate medication management across telephone, outpatient clinic, and home visits.
• Precept pharmacy residents, students and interns in value-based clinical programs.
• Collaborate with physicians and prescribers to optimize therapies.
