• Pharm D preferred; must be able to be licensed in SC, GA and FL.
• Clinical Pharmacist role supports the care team to optimize patient outcomes through safe medication use.

Responsabilités

• Review patient records to determine the appropriateness of medication therapy.
• Evaluate patient condition to ensure issues are addressed and treated.
• Develop medication recommendations to minimize risk of adverse effects and ensure proper drug therapy.
• Counsel patients on appropriate medication use and assess barriers to adherence.
• Collaborate with healthcare professionals to ensure optimal patient care.
• Document medication plans and patient progress in the medical record and ensure proper billing of visits.
• Interact with patients via phone, email, patient portal, in person, etc. to gather necessary information.
• Provide reasonable accommodations as required by law to enable performance of duties.
