Your Health Organization in Columbia, SC is seeking a Pharmacist to join our urgent care team. The role focuses on optimizing medication therapy, patient counseling, and collaboration with healthcare professionals to improve patient outcomes.

The Clinical Pharmacist will review records, provide recommendations, document progress, and support billing and care coordination. Pharm D preferred and licensure in SC, GA, and FL is required or expected.

Qualifications

• Pharmacist duties include optimizing med therapy and patient outcomes.
• Experience with medication reviews and counseling is preferred.
• Ability to collaborate with healthcare teams to ensure optimal care.

Responsibilities

• Review patient records to ensure appropriate medication therapy.
• Evaluate patient conditions to adjust therapies and minimize adverse effects.
• Develop medication recommendations and educate patients as per care plans.
• Counsel patients on medication use and adherence strategies.
• Collaborate with clinicians to ensure high quality patient care and proper billing.
• Document medication plans and progress in medical records; interact with patients via multiple channels.
