What Are We Looking For?
Your Health is currently seeking a Pharmacist to join our growing urgent care team. The Clinical Pharmacist is a healthcare professional who works as part of the care team to optimize patient outcomes through safe and effective medication use. Responsibilities include providing patient consultation, recommendations, and education according to the patient’s care plan. A successful Pharmacist will review patient records to determine medication appropriateness, evaluate patient conditions, develop medication recommendations to minimize risks, counsel patients on medication use, assess barriers to adherence, collaborate with healthcare professionals, document medication plans, and interact with patients through various communication channels.
