Job summary

Your Health seeks a Clinical Pharmacist to join the urgent care team. The Pharmacist will review patient records, provide evidence-based medication recommendations, counsel patients, and document progress in the medical record. A PharmD is preferred, with licensure in SC, GA, and FL or active pursuit of licensure. The role involves collaboration with clinicians and direct patient interactions across facilities, homes, clinics, and virtual visits.

Qualifications

• PharmD preferred and ability to be licensed in SC, GA and FL.
• Clinical Pharmacist provides patient consultation, recommendations, education per plan of care.
• Responsible for documenting medication plans and patient progress.

Responsibilities

• Review records to determine appropriateness of medication therapy.
• Develop medication recommendations to minimize adverse effects.
• Counsel patients on medication use and address adherence barriers.
