The University of Kansas Health System is seeking a Clinical Pharmacist – Transitions of Care at Bell Hospital. You will provide comprehensive pharmaceutical care during admissions and discharges, ensuring safe, effective, and cost-conscious medication therapy with strong interdisciplinary collaboration.

Responsibilities include rounding with the team, adjusting doses for renal/hepatic function, reconciling meds on admission/discharge, and educating staff and learners.

Qualifikationen

• Bachelors Degree in Pharmacy from an accredited college or university.
• Completion of an accredited pharmacy post graduate training program (PGY-1 and/or PGY-2 residency).
• Licensed Pharmacist - State Board of Pharmacy in state of primary practice site.
• ACLS, PALS/NRP, and BLS certifications as necessary.
• Board of Pharmaceutical Specialties certification in chosen area of specialty (preferred).

Aufgaben

• Participates in performance improvement efforts to strengthen systems and patient care.
• Attends rounds with the multidisciplinary team providing patient-focused pharmacotherapy.
• Supports staffing by flexing to other areas or shifts as needed.
• Ensures timely medication order processing and resolution of issues.
• Performs patient-specific medication profile reviews with PK dosing and IV to PO conversions.
• Adjusts doses for renal or hepatic dysfunction.
• Reports medication incidents and participates in safety reviews.
• Participates in medication reconciliation and patient education.
• Precepts residents and students; provides feedback and mentorship.
• Shares expertise through educational presentations and in-services.
