Job summary

Bell Hospital seeks a Clinical Pharmacist - Transitions of Care to provide comprehensive pharmaceutical care for patients during transitions such as admission and discharge, ensuring safe, effective, and cost-conscious drug therapy.

The role requires strong interdisciplinary communication and the ability to educate and precept learners, participate in rounds, and support medication reconciliation and dosing adjustments to optimize patient safety.

Qualifications

• Bachelor's degree in Pharmacy from an accredited college or university.
• Licensed Pharmacist – State Board of Pharmacy.
• ACLS, BLS, and PALS/NRP certifications as applicable.
• PGY-1/PGY-2 residency training preferred.

Responsibilities

• Actively participates in performance improvement efforts to strengthen systems, processes, policies and procedures.
• Attends patient care rounds and provides patient-focused pharmaceutical care by assessing and monitoring drug therapy.
• Supports staffing needs by assisting in supplemental shifts when needed.
• Ensures timely medication order processing by participating in order entry, verification and review of orders.
• Performs patient-specific medication profile reviews and adjusts doses for renal or hepatic dysfunction.
• Precepts residents and students, providing education, feedback and mentorship.
