Responsibilities

• Reviews and verifies medication orders with patient-specific parameters (diagnosis, weight, age, allergies, organ function, labs, interactions).
• Documents and communicates pertinent information with the healthcare team to ensure continuity of care.
• Provides drug information to physicians, nurses and other health care professionals.
• Supervises students, technicians and residents and checks medications such as compounded sterile products and unit-dose meds as needed.
• Promotes patient safety by reporting medication events and near misses.
