Job summary

UChicago Medicine seeks a Clinical Pharmacist Specialist to provide expert care across ambulatory and hospital settings, including primary care and weight loss clinics. You will work within interdisciplinary teams to optimize medication use, implement pharmaceutical interventions, and educate patients and staff.

The role emphasizes direct patient-centered care, safety-focused medication management, and participation in stewardship programs within a major academic health system in Chicago.

Qualifications

• Graduate of an ACPE accredited College of Pharmacy.
• Completion of ASHP accredited PGY-1 Pharmacy Residency.
• Eligible for licensure as a pharmacist in Illinois (within 60 days).
• BPS Certification or pharmacy-related certification within 18 months of hire (e.g. BCPS, BCOP, BCACP, etc.).

Responsibilities

• Maintains proficiency in clinical pharmacy programs and operations.
• Prioritizes patient-centered care in all actions.
• Collaborates with multidisciplinary teams to optimize medication use and patient outcomes.
• Performs comprehensive medication histories and reconciliations.
• Provides discharge medication review and patient education.
