Job summary

Qualifications
- Bachelor of Science in Pharmacy is required.
- Illinois Pharmacist License required.
- Minimum 4 years of clinical pharmacy experience OR PGY1 residency within last 7 years.
- Knowledge of IV admixture, IV drug therapy and chemotherapy.
- Ability to work effectively with others to provide safe drug therapy.
- Strong problem-solving skills and independent work style.
Responsibilities
- Verify orders, discharge counseling, and education of healthcare staff.
- Evaluate physician orders for dosing, interactions, and safety.
- Clarify orders with prescribers when needed and communicate recommendations.
- Provide pharmacotherapy recommendations to medical/nursing staff.
- Document clinical interventions and monitor pharmacokinetics.
- Oversee pharmacy technicians and coordinate daily medication workflows.
- Participate in medication safety initiatives and formulary decisions.
