Qualifications

• Graduate of an ACPE-accredited College of Pharmacy.
• At least five years of clinical pharmacy experience with 2 years in a specialty area, or completed a residency with a specialty component.
• Board Certification in Pharmacotherapy or equivalent advanced degree with residency.
• Proficiency with pharmacy information systems and education/training for providers.

Responsibilities

• Participate in rounds with the care team and provide therapeutics education.
• Review and monitor drug therapy orders and adverse reactions; optimize therapy.
• Contribute to formulary decisions and develop evidence-based protocols.
• Precept pharmacy students and residents; collaborate with physicians and other clinicians.
